Centene's health plans share a steadfast commitment to providing meaningful, accessible, high-quality healthcare to every member. As part of Centene's local activation approach, its health plans are also designed to deliver locally-based services benefiting from enhanced collaboration and strategic coordinated care programs. The combination of all of these elements helps improve the quality of life for millions of members every day. Recently, one of our young members with Centene's Missouri plan, Home State Health, was in dire need of coordinated care services.

Home State Health provides a wide range of healthcare services, but coordinated care addresses our members' most complex healthcare needs while taking a "whole health" approach to improving outcomes. Care coordination involves working with providers and caregivers to share important clinical information and have clear, shared expectations about their roles. Keeping patients and their families informed is equally important, making sure that records are kept current and accessible.

When Home State Health member Jayden was diagnosed with a serious health condition, he underwent chemotherapy and suffered debilitating side effects while he received multiple avenues of care. Home State Health Case Manager Nichole met with Jayden and his family, and immediately saw a need for coordinated care services. With Nichole's help, Jayden's care was streamlined, focusing on his whole health.
